Healing is a bit different in leveling than most skills. You get only one "Heal" in a sense to Hard HP per, hmm, let's call it a damage instance. So to be able to do a basic "Heal" on them again, they need to take more Hard HP damage.

Some folks call it a bug, I have heard it as "working as intended." Later when bandaging is in the game, that will likely turn into the true Healing of the game. (Edited to add: Err, not bandaging sorry, First Aid is the skill set, bandage self is one of the abilities within it.)

Once you get to level 30, you are able to Heal wounds on limbs. As in a cut on the arms or legs that provides a debuff of the player. At 60 you are able to Heal fractures, again on limbs.

It's not until level 90 that you are capable of healing wounds and fractures on all body parts, and this is key, because that also includes the Torso. Most wounds and fractures end up on the Torso, for two reasons. First, it's the biggest and easy part of a person to damage. And second, because animals only ever target a player's Torso. Sure, they can occasionally do arm or leg damage, but that's normally due more to the player's actions than the animal's.
