patch.sql on dedicated server setup. what to do?

When setting up a new server do you Query the patch.sql? I have seen some people say Query it and some people say it gets run when server starts.

Every time the server starts patch.sql gets quiried too.

thanks! I actually see it now. After some issues trying to get a second server running, I see that it wrote it to the DB.
