So, I was grinding away picking some mushrooms; I looked up and noticed a red icon at the top of the screen. Thinking, "red is bad," I looked at it and it told me I was wounded. I thought, "huh?!" Nothing attacked me, I was just here rooting for mushrooms. Looking into it, I apparently had a wound on my left forearm... and a few HardHP gone. When I tried to bandage, it said I didn't have any wounds to bandage. It did eventually heal, but it confused me. Can you get wounded from simply grinding away?

You can be wounded and also killed. Here's some of the more fun ones:

Carpal Tunnel: Minor hand wound, negligible hard hp loss. Cause while performing tasks requiring manual dexterity.
Sprained ankle: Foot wound, negligible hard hp/stam loss, hobbled. Cause while carrying objects.
Smacked by a branch: Head wound. Moderate hard hp loss, possible bleeding. Caused during logging.
SNAKE!: Go to dead. Go directly to dead. Do not pass GO!. Do not collect 200 copper. Hard hp loss anywhere between 50% and whatever the hell you had left. Occurs in forest and grasslands and can happen pretty much anywhere for any reason. If you're lucky you will just be severely wounded. If not, I hope you bound to a home.

I've actually been smacked by a branch, but in that case, one of those descriptive pop-ups came up, so I knew what was going on. In this case, I didn't see one... or maybe I missed it.

I have been hit by a branch, caused my screen to go all wonky for a few minutes.

Hit by my shovel.... straight up KO'ed me.

Been struck by lightning... killed me out right (this was in CBT4).

Bit by a snake, nearly killed me.

Lost my footing, fell on my butt.

Sprained my ankle... slowed me down and caused a leg wound.

Found a 100ql knife.

Found a 100ql bolt

Broke my arm chopping down a tree...

Yep... sometimes the gods dont look favorably on us.

we had a guy get stung by a bee on his eye... did hp damage like a snake bite plus made his screen go wonky like when your drunk lol.

Unrelated note: Found large magnet lost weeks ago behind monitor

I get bit by snakes all the time and lose 50% of my health.... I live in the snow. That being said I do really love the idea of these "random events" even though some of them are pretty silly.
